Overview

We're looking for a compassionate, organized, and resourceful care navigator to support individuals in accessing health, wellness, and social support services.

In Short

  • Serve as the main point of contact for clients seeking care or support
  • Help clients understand and access available resources (medical, behavioral, financial, etc.)
  • Coordinate appointments, referrals, follow-ups, and service navigation
  • Maintain accurate documentation of client interactions and status updates
  • Communicate with care providers, case managers, and support teams
  • Follow up with clients to ensure needs are met and barriers are addressed
  • 2+ years of experience in care coordination, case management, social services, or customer support
  • Strong interpersonal and communication skills
  • Empathetic, solution-focused, and able to work with clients from diverse backgrounds
  • Ability to work independently in a remote environment

Requirements

  • U.S. citizenship required
  • Organized and tech-savvy; comfortable using systems like Salesforce, EHRs, or CRM tools
  • Ability to manage multiple clients
