Care Navigator - Remote

Overview

Boulder Care is looking for a Care Navigator for a 100% remote, full-time position to work a 9am - 6pm PST shift.

In Short

• Support current patients seeking Substance Use Disorder (SUD) care.
• Answer phone calls regarding pharmacy issues, rescheduling visits, and app issues.
• Utilize problem-solving skills to resolve non-standard questions.
• Document patient interactions using SBAR.
• Conduct targeted patient outreach based on company initiatives.
• 3+ years of experience in a healthcare setting required.
• Knowledge of healthcare terminology, especially addiction medicine.
• Exceptional customer service and de-escalation skills needed.
• Must be comfortable with technology and common business tools.
• Team-focused and collaborative environment.
