Overview

As a Care Navigator at Optimize Health, you will build relationships with patients to promote positive health behavior changes and improve patient outcomes through remote monitoring and communication.

Requirements

• Clinical background with adult and geriatric populations.
• Experience in phone triage.
• Strong knowledge of remote patient monitoring (RPM).
• Familiarity with Chronic Care Management (CCM) principles.
• Understanding of Behavioral Health and Care Coordination.
• Ability to work as a strong team player.
• Desire to support patients' health and well-being.
• Experience with remote care platforms.
• Strong communication skills.
• Ability to monitor and communicate effectively with patients.
