As a care team member, you will build relationships with patients based on trust, utilizing motivational communication techniques to help drive positive health behavior change and improved patient outcomes.

Requirements

• Clinical background with adult and geriatric patients.
• Experience in phone triage is ideal.
• Strong knowledge of remote patient monitoring (RPM) preferred.
• Familiarity with Chronic Care Management (CCM) principles.
• Understanding of Behavioral Health and Care Coordination.
• Ability to work remotely and be a strong team player.
• Desire to support patients clinically and emotionally.
• Attention to reimbursement requirements.
