Care Team Member - LPN/LVN - Remote

Overview

As a Care Team Member at Optimize Health, you will build relationships with patients to drive positive health behavior change through remote monitoring and support.

• Build relationships with patients based on trust.

• Conduct wellness calls with patients.

• Follow escalation pathways for urgent care needs.
• Work remotely within the PST time zone.

Requirements

• Unrestricted LPN/LVN license in a compact state.
• Unrestricted LPN/LVN license in CA, NV, or OR.
• 1-3 years of clinical experience preferred.
• Experience with telephonic care management.
• Ability to work independently.
• Spanish speaking preferred.
• Experience with Medicare patients.
• Interest in professional growth opportunities.
• Health Coach certification a plus.
• Motivational Interviewing experience a plus.
