Care Navigator - LPN/LVN - Remote

Overview

As a Care Navigator at Optimize Health, you will build relationships with patients, using motivational communication techniques to drive health behavior changes and improve outcomes through remote patient monitoring.

In Short

• Manage patient physiological markers and provide education.
• Conduct wellness calls to define health goals.
• Collaborate with care teams for patient management.
• Utilize remote care platforms for patient interaction.
• Work independently with minimal direction.
• Assist with disease management and preventive health education.
• Engage with patients to improve health outcomes.
• Experience with Medicare patients preferred.
• Spanish speaking is a plus.
• Unrestricted LPN/LVN license required.

Requirements

• 1-3 years of clinical experience preferred.
• Ability to work EST hours.
• Experience with virtual visits and telephonic care management.
• Interest in professional growth and process development.
• Motivational Interviewing experience is a plus.
• Health Coach certification is a plus.
