Care Navigator - Remote

Overview

As a Care Navigator at Remo, you will play a vital role in changing the care journey for individuals living with dementia and their caregivers. You will provide compassionate support and coordinate care while working with a multidisciplinary team.

In Short

• Build trusting relationships with individuals living with dementia and their caregivers.
• Provide empathetic support and education for Dyads.
• Collaborate with the care team to meet quality targets.
• Act as a liaison between Dyads and local in-home partners.
• Participate in care team huddles and conferences.
• Manage care coordination and provide caregiver support.
• Support Dyads through diagnosis and transitions of care.

Requirements

• Experience working with older adults, especially those with cognitive disorders.
• Based in Cleveland, Ohio.
• Experience in multidisciplinary care teams.
• Excellent active listening and organizational skills.
• Passion for educating families and clinical providers.
• Strong problem-solving skills and creativity.
• Ability to work independently in a remote setting.
