Overview

As a Care Navigator, you will be responsible for providing patient education and navigation of programs and services during new member onboarding within the first 90 days of membership.

In Short

  • Manage a caseload of new members within their first 90 days of membership.
  • Handle a high volume of inbound and outbound calls.
  • Conduct welcome calls to new members and provide program introductions.
  • Follow up after new patient appointments to assist with medication and referrals.
  • Educate patients on policies and responsibilities of IMA Medical Group.
  • Introduce patients to the Patient Portal for accessing information.
  • Direct patients to appropriate urgent care services.
  • Retrieve patient records from Hospital Portals as needed.

Requirements

  • High school diploma or GED.
  • 1-2 years of primary care experience.
  • Proficient knowledge in medical terminology.
  • Effective interpersonal and phone skills.
  • Exceptional customer service skills.
  • Bilingual in Spanish.
