RN Care Navigator - Remote

Overview

The Registered Nurse, Care Navigator will be responsible for case management specific to kidney health management, facilitating high quality, cost-effective outcomes for patients while coordinating care delivery.

In Short

• Handle in and outbound calls delivering world-class service to members
• Educate on kidney health and related co-morbid conditions
• Engage members into HealthMap’s Kidney Health Program
• Follow up with members based on complexity and cadence by policy
• Serve as patient advocate for resolving concerns or barriers
• Utilize community resources in care planning
• Coordinate care with interdisciplinary teams
• Notify providers of identified patient needs
• Comply with HIPAA and other regulations
• Accountable for individual metrics and KPIs

Requirements

• Active, unrestricted RN license required
• Bachelor's degree required
• CCM preferred
• Three years of case management experience preferred
• Experience in a dialysis or transplant center preferred
• Experience with Medicare and Medicaid preferred
