Overview

The Social Worker is a behavioral health professional providing outreach to members, assisting in identifying social determinants of health and linking them to appropriate community resources while collaborating with a Clinical Care Team.

In Short

• Complete Behavioral Health Assessments
• Conduct telephonic outreach for member engagement

Requirements

• Master’s degree in social work required
• LCSW license preferred
• 3 years of experience in healthcare or human services
• Experience in managed care setting
• Knowledge of renal health and community resources preferred
• Must reside in Michigan
