Overview

The Social Worker is a behavioral health professional providing outreach to members, assisting in identifying social determinants of health, barriers to care, and linking to appropriate community resources.

In Short

• Complete Behavioral Health Assessments to identify psychosocial needs.
• Assist in telephonic outreach for member engagement.
• Accountable for individual metrics defined by leadership.
• Provide actionable interventions and supportive counseling.
• Facilitate linkage to community health services.

• Collaborate with outside agencies for continuity of care.
• Participate in team and 1:1 meetings.
• Document in HealthMap’s Care Management Systems.

Requirements

• Master’s degree in social work required.
• LCSW license preferred.
• 3 years experience in healthcare or human services.
• Experience in managed care setting.
• Knowledge of renal health/community resources preferred.
• Must reside in NYC.
