Requirements

• Master’s-level licensure in relevant fields (LCSW, LCMHC, LPA, LMFT, RN).
• Three years of experience in care management or coordination.
• Proficient understanding of person-centered care and compliance.
• Strong interpersonal and communication skills.
• Ability to make independent decisions based on facts.
• Skilled in conflict management and resolution.
• Proficient in Microsoft Office products.
• Knowledge of Medicaid and relevant federal laws.
• Experience with Motivational Interviewing.
• Ability to travel to community locations as needed.
