Overview

The CAP Case Manager provides advanced social work services for the Community Alternatives Program for Disabled Adults and Children, assisting in locating, coordinating, and monitoring services to meet patient needs.

In Short

  • Conduct assessments and evaluations of patients and families.
  • Develop and update care plans for patients.
  • Provide counseling and emotional support to patients and families.
  • Collaborate with other disciplines to ensure comprehensive patient care.
  • Maintain documentation and records per agency policy.
  • Participate in interagency conferences.
  • Complete continuing education and training requirements.
  • Flexibility in scheduling patient visits and documentation.
  • Use personal vehicle for home visits and receive mileage reimbursement.
  • Work in a hybrid model combining remote work and in-person visits.

Requirements

  • Bachelor’s degree in social work or related field.
  • Experience in community services or case management.
  • Knowledge of social work principles and community resources.
  • Strong organizational and communication skills.
  • Ability to travel for home visits.
