Overview

The CAP Case Manager at Healthkeeperz is responsible for providing essential case management services for the Community Alternatives Program (CAP) for Disabled Adults and Children, ensuring participants receive the necessary medical, social, and financial services to live independently.

In Short

  • Conduct comprehensive assessments and evaluate eligibility for CAP services.
  • Develop individualized care plans tailored to each patient’s needs.
  • Collaborate with patients, families, and provider agencies.
  • Perform regular home visits and telephone check-ins.
  • Maintain accurate documentation in accordance with agency guidelines.
  • Provide ongoing monitoring and evaluation of care effectiveness.
  • Coordinate care with interdisciplinary team members.
  • Participate in professional development and training.
  • Use personal vehicle for travel to patient visits.
  • Work in a hybrid model combining remote work and field responsibilities.

Requirements

  • Bachelor’s degree in social work or related field.
  • One year of community experience in case management preferred.
  • Strong organizational and communication skills.
  • Knowledge of social work principles and community resources.
  • Ability to travel for home visits.
