The TCM Care Manager position involves providing whole-person care management for beneficiaries in the Behavioral Health I/DD Tailored Plan, focusing on assessment, care planning, and coordination of services.

Requirements

• Bachelor’s degree in a relevant field or RN licensure.
• Two years of experience with behavioral health conditions.
• Proficiency in Motivational Interviewing techniques.
• Strong interpersonal and communication skills.
• Ability to prioritize and meet deadlines.
• Experience with care management for LTSS needs preferred.
• Knowledge of Medicaid benefits plans.
• Proficiency in Microsoft Office.
• Conflict management and resolution skills.
• Ability to make independent decisions.
