CAP Case Manager - Hybrid Remote

Overview

Healthkeeperz is hiring a CAP Case Manager to support the Community Alternatives Program (CAP) for Disabled Adults and Children, providing assessment, care planning, coordination, and ongoing monitoring to help individuals safely remain and receive care in their homes.

In Short

• Full-time position with hybrid remote work options.
• Starting pay of $18.75 – $20.00 per hour.
• Conduct assessments to determine CAP eligibility.
• Develop individualized care plans based on needs.
• Coordinate services with families, providers, and agencies.
• Perform home visits and follow-up monitoring.
• Maintain accurate documentation in E-CAP.
• Ensure compliance with CAP and state program guidelines.
• Participate in required training and education.
• Flexible scheduling with up to 2 remote workdays per week.

Requirements

• Bachelor’s in Social Work + 1 year related experience.
• Bachelor’s in Human Services + 2 years related experience.
• Bachelor’s in Non-Human Services + 2 years related experience.
• Current NC RN license + 1 year case management experience.
• Apprentice/Trainee (Bachelor’s or RN without full experience).
• Knowledge of casework and care coordination practices.
• Familiarity with community resources.
• Strong communication, time management, and organization skills.
• Ability to travel for visits and meetings.
• Proficiency with electronic documentation tools.
