Overview

The Care Manager is part of a multidisciplinary care team providing whole-person care management for Behavioral Health I/DD Tailored Plan beneficiaries, focusing on various health domains.

In Short

• Conduct comprehensive assessments and develop care plans.
• Utilize person-centered planning and motivational interviewing.
• Coordinate with team members for smooth transitions of care.
• Educate members about care services and options.
• Ensure compliance with service orders and documentation.
• Evaluate service appropriateness and escalate complex cases.
• Assist individuals in choosing service providers.
• Collaborate with care teams to develop individualized plans.
• Maintain certifications and comply with agency policies.
• Participate in 24-hour coverage for care management.

Requirements

• Bachelor’s degree in health-related field or RN licensure.
• Two years of experience with behavioral health or I/DD.
• Experience in LTSS and HCBS coordination for relevant roles.
• Proficiency in person-centered thinking and planning.
• Strong communication and conflict resolution skills.
• Ability to make independent decisions and prioritize tasks.
• Familiarity with Medicaid benefits and care management.
• Proficiency in Microsoft Office products.
