In Short

• This Supervisor provides regular administrative and clinical supervision to Case Managers and RNs.
• Conduct comprehensive assessments and create care plans.
• Coordinate in-home services via a network of vendor agencies.
• Train staff and provide timely feedback on performance.
• Ensure services are delivered in compliance with program regulations.

Requirements

• Bachelor's Degree required.
• Minimum 2 years of experience in social services field.
• Supervisory experience preferred.
• Master's Degree in Social Work or related field preferred.
• A reliable vehicle, current driver's license and safe driving record required.
