Overview

This is a temporary position focused on supporting the Medicare Dual Special Needs Plan (D-SNP) Care Management team with service coordination for members.

• Supports D-SNP Care Management team with coordination of services.

• High school diploma or equivalent required.
• Three years of experience in a health care setting preferred.
• Hybrid work environment with remote interviews.
