Temporary Care Coordinator - Remote

Overview

The Temporary Care Coordinator will provide care coordination services to individuals enrolled in the One Care program, working collaboratively with a team to ensure patients are linked to necessary services and improve their health outcomes.

In Short

• Full-time position, Monday – Friday 8:30-5:00pm.
• Collaborate with medical teams to support patients with complex health needs.
• Engage patients through telephonic and face-to-face outreach.
• Support administrative functions for the One Care team.
• Assist with housing stability resources for patients experiencing homelessness.
• Conduct outreach to maintain patient connections to care.
• Assist in arranging patient appointments and accompany them if needed.
• Perform referrals and service authorizations for patients.
• Flexible in duties as the role expands.
• Position funded until the end of December 2025 with possible renewal.

Requirements

• Bachelor's degree preferred.
• Experience in case management or human services preferred.
• Proficient in electronic medical record documentation and Microsoft Excel.
• Bilingual English-Spanish preferred.
• Strong problem-solving and communication skills.
• Excellent customer service skills.
• Detail-oriented and organized.
• Self-directed and able to work independently and in a team.
• Able to triage patient needs effectively.
