Care Coordinator, BH-CP (Hybrid - In Person/Remote)

Overview

The Care Coordinator at BHCHP will work with a multidisciplinary team to provide high-quality care for homeless individuals with complex health needs. This role includes both in-person and remote responsibilities.

In Short

• Full-time position, Monday to Friday, 8:30am - 5:00pm
• Hybrid work model (in-person/remote)
• Engage with enrollees to connect them with necessary health services
• Conduct assessments and document progress in a data platform
• Assist with appointment arrangements and care coordination
• Participate in team case conferences
• Advocate for enrollees’ needs
• Request and send medical records as needed
• Flexible and able to multi-task
• Work with vulnerable populations

Requirements

• Bachelor's degree in social work or related field, or equivalent experience
• Excellent customer service and communication skills
• Detail-oriented and organized
• Ability to work independently and as part of a team
• Experience with motivational interviewing and cultural competency is a plus
• Preferred experience in case management or human services
• Proficient in Microsoft Excel and data entry
• Must be flexible and able to multi-task
