Care Coordinator (Hybrid/Remote)

Overview

The Care Coordinator will provide care coordination services to individuals enrolled in the One Care program, ensuring that patients are linked to needed services to improve their health and overall well-being.

In Short

• Collaborate with One Care team and primary care teams.
• Engage patients through telephonic and face-to-face outreach.
• Maintain communication with Managed Care Organization (MCO).
• Assist with housing process for patients experiencing homelessness.
• Conduct outreach to shelters and patients’ homes.
• Assist nurse care managers with patient appointments.
• Perform referrals and service authorizations for patients.
• Remain flexible in duties as role expands.

Requirements

• Bachelor's degree preferred.
• Experience in case management or human services preferred.
• Computer skills with electronic medical records and Microsoft Excel.
• Bilingual English-Spanish preferred.
• Strong problem-solving and communication skills.
• Excellent customer service skills.
• Detail-oriented and organized.
• Self-directed with ability to work independently and as part of a team.
• Ability to triage patients’ needs creatively.
