As a Hybrid Care Coordinator at Plume, you will play a crucial role in ensuring high levels of patient care and satisfaction in a telehealth environment, while also providing in-person support at various clinics in Florida.

In Short

• Be the first point of contact for patients in Florida.
• Ensure high levels of patient care and satisfaction.
• Answer current and prospective patient questions.
• Triage inquiries to appropriate medical staff.
• Ensure accurate data flow.
• Support members' labs, pharmacy, and resource needs.
• Facilitate peer support groups as services grow.
• Work in a hybrid model with both remote and in-person shifts.
• Travel up to 50% throughout Florida.
• Work at least one weekend per week.

Requirements

• Strong understanding of the trans experience.
• 2+ years in a customer service role.
• Attention to detail with data management skills.
• Empathetic communication skills.
• Basic computer skills and ability to learn new systems.
• Hybrid in-person care coordination experience.
• Ability to travel for work.
