• Seeking a Clinical Lead Care Manager (CLCM) for Sacramento County, CA.
• Primary contact for clients, coordinating with healthcare providers.
• Manage client cases and facilitate access to care.
• Hybrid position requiring travel throughout Sacramento County.
• Assess member needs in various health areas.
• Advocate for clients with healthcare professionals.
• Utilize evidence-based practices in care management.
• Maintain patient health records and complete documentation.
• Conduct outreach and engagement activities.
• Provide mental health promotion and arrange transportation.

Requirements

• Active California Licensed Vocational Nurse (LVN) license required.
• 2+ years of experience in care management or related fields.
• Bilingual, Spanish/English, required.
• Excellent communication and interpersonal skills.
• Working knowledge of social determinants of health.
• Clean driving record and valid driver's license.
• General computer skills and knowledge of Google Workspace and MS Office.
• Willing to work Monday-Friday 8:30am-5:00pm, both in field and remotely.
