In Short

• Assess member needs in various health areas.
• Oversee the development of client care plans.
• Provide services in various settings including telehealth.
• Connect clients to needed social services.
• Advocate for clients with health care professionals.
• Utilize evidence-based practices in care management.
• Conduct outreach and engagement activities.
• Evaluate client progress and update goals.
• Maintain up-to-date patient health records.
• Complete monthly reporting for program compliance.

Requirements

• Active California Licensed Clinical Social Worker (LCSW) or Licensed Marriage and Family Therapist (LMFT) license required.
• 1+ year of experience in care management or similar roles.
• Excellent communication and interpersonal skills.
• Clean driving record and reliable transportation.
• General computer skills and knowledge of Google Workspace and MS Office.
• Bilingual (English/Spanish) is a plus.
