Clinical Care Coordinator - Remote

Overview

The Clinical Care Coordinator is responsible for providing personalized access to behavioral health and emotional support, ensuring effective communication between participants and providers throughout the scheduling process.

In Short

• Secures appointments with providers
• Keeps participants updated during the scheduling process
• Provides necessary paperwork to participants and providers
• Conducts telephonic assessments and follow-ups
• Reviews and collaborates on action plans with providers
• Acts as a liaison for participants navigating health insurance
• Troubleshoots participant issues as they arise
• Researches and qualifies referrals to community resources
• Documents all case activities in the data management system
• Participates in required on-call and Saturday rotations

Requirements

• Masters’ degree in Social Work or Psychology required
• 2-3 years of call center experience required
• 2-3 years of experience in Employee Assistance or Well-Being Programs preferred
• 2-3 years of case management experience preferred
• Proficient in Microsoft Word, Excel, and Outlook
