The Clinical Care Coordinator is responsible for providing personalized access to behavioral health and emotional support, ensuring effective communication and coordination between participants and providers.

• Secures appointments with providers
• Keeps participants updated on appointment progress
• Provides intake and assessment information
• Follows up with providers and participants
• Reviews and collaborates on action plans
• Advocates for participants navigating health insurance
• Troubleshoots participant issues
• Researches community resources
• Documents all case activities
• Participates in on-call and Saturday rotations

Requirements

• Masters’ degree in Social Work or Psychology
• 2-3 years of call center experience
• 2-3 years in Employee Assistance or Well-Being Programs preferred
• 2-3 years in case management preferred
• Proficient in Microsoft Word, Excel, and Outlook
