Overview

The Care Team Manager role holds a supervisory scope of work while balancing a small member portfolio, which reflects the work of the Community Guides at a leadership capacity. This role is focused on supporting our mission-driven, member-focused company by leading a team of Community Guides and ensuring effective communication and collaboration.

In Short

  • Supervises a team of Community Guides.
  • Leads and manages remote Care Teams.
  • Facilitates virtual meetings and communications.
  • Provides training and development for new staff.
  • Engages with members through various communication methods.
  • Accountable for Care Team Program metrics and goals.
  • Supports community outreach initiatives.
  • Ensures compliance with HIPAA and confidentiality policies.
  • Participates in the interview process for new hires.
  • Encourages member independence and well-being.

Requirements

  • Bachelor’s degree in Social Work, Human Services, Psychology, or related fields.
  • Three years of program management/supervision experience.
  • Five years of experience with the local population.
  • Strong critical thinking and time management skills.
  • Knowledge of crisis planning and case management.
  • Proficiency in building relationships quickly.
  • Experience with outreach plans and community resources.
  • Familiarity with Microsoft Office and video conferencing tools.
  • Must be 18 years of age or older.
  • Valid Driver's License and reliable transportation required.
