Care Manager - Remote

Overview

The Care Manager plays a critical role in transforming the health of communities by developing and facilitating complex care management activities primarily for mental and behavioral health needs. This position focuses on providing high-quality, cost-effective healthcare outcomes through personalized care plans and education for members and their families.

• Develops and assesses care management activities for members.
• Creates personalized care plans related to mental health and substance use disorders.
• Conducts evaluations via phone or in-home visits.

Requirements

• Master's degree in Behavioral Health or Social Work or a nursing degree.
• 2-4 years of related experience in care management.
• Licensed Master's Behavioral Health Professional or RN as per state requirements.
• Experience in behavioral health required.
• For Sunshine Health (FL): Master’s degree in social work with pediatric care experience.
• Ability to travel locally up to 80%.
