Overview

The Care Manager (RN) position involves working with members and healthcare teams to provide integrated care for patients with high needs in New York State. The role focuses on assessing, planning, and coordinating care to ensure quality outcomes.

Requirements

• Graduate from an Accredited School of Nursing.
• 1-3 years in relevant healthcare settings.
• Active RN license in good standing.
• Valid driver's license and reliable transportation.
• Bachelor's Degree in Nursing preferred.
• 3-5 years of experience preferred.
• Certified Case Manager (CCM) certification preferred.
