Overview

The Health Home Care Coordinator is responsible for assessing, facilitating, planning, and coordinating integrated care delivery for members with high needs, ensuring quality and cost-effective care.

In Short

• Work with members and multidisciplinary teams to coordinate care.
• Support behavioral health and long-term care services.
• Conduct clinical assessments and develop care plans.
• Perform outreach calls to educate and enroll potential members.

Requirements

• 1-3 years experience in case management or related fields.
• Completion of an accredited LVN/LPN program or relevant degree.
• Active and unrestricted license if required.
• Valid driver's license and reliable transportation.
• Preferred experience includes 3-5 years in similar settings.
• Preferred certifications in social work or counseling.
