Overview

The Healthcare Coordinator is responsible for providing exceptional patient service and care coordination across disciplines, ensuring the best clinical care for patients.

In Short

• Perform telephonic outreach to enroll and engage patients.
• Develop and implement care plans under the direction of Nurse Care Managers.
• Conduct assessments to identify social determinants impacting health.
• Guide members in utilizing health and social services.
• Advocate for members by identifying barriers to care.
• Maintain communication with patients and healthcare teams.
• Ensure accurate documentation of member interactions.
• Provide educational resources to empower patients.
• Work effectively in a remote environment.
• Other duties as assigned.

Requirements

• 3 years of experience in healthcare coordination or similar field.
• Experience with a team-based approach to care.
• Excellent knowledge of community resources.
• Strong prioritization and task-switching skills.
• Self-directed and motivated problem solver.
• Strong communication and teaching skills.
• Intermediate skills with MS Office Suite.
• Ability to work effectively in a remote environment.
