Requirements

• Bachelor’s Degree in Nursing preferred; or Associate Degree with relevant experience.
• Licensed as RN, LCSW, OT, RT, certified CRC, CDMS, or any other license.
• At least 2 years of experience in case management or care coordination.
• Skilled in developing and evaluating individualized care plans.
• Excellent verbal and written communication skills.
• Strong analytical and problem-solving abilities.
• Ability to build and maintain positive relationships.
• Exceptional organizational and time management skills.
• Proficient in using electronic health records (EHR).
