Overview

The Care Manager is responsible for planning and managing behavioral and physical care for members, ensuring coordination of health care services to enhance member experience and health outcomes.

In Short

  • Advocates and educates members on health services and self-management techniques.
  • Conducts assessments to identify barriers and opportunities for intervention.
  • Develops care plans aligned with physician treatment plans.
  • Generates referrals to appropriate providers and community resources.
  • Liaises between service providers to ensure coordinated care.
  • Coordinates with multi-disciplinary health care teams.
  • Evaluates service authorizations for alignment with care plans.
  • Documents all encounters in compliance with procedures.
  • Handles occasional overtime as necessary.
  • Additional duties as assigned.

Requirements

  • NYS RN or LCSW or LMSW required for Medical Care Management.
  • 3 years of experience in mental/behavioral health or addictions setting.
  • Experience with pediatric clinical fields for PEDS positions.
  • Strong interpersonal and assessment skills.
  • Fluency in Spanish, Korean, Mandarin, or Cantonese preferred.
  • Experience with community health practices for frail adults.
  • Ability to manage large caseloads effectively.
  • Proficient with electronic documentation systems.
  • Experience using Microsoft Excel.
