Overview

The Care Coordinator is responsible for performing care management services to assist members in navigating their healthcare system, providing excellent customer service to NorthStar Members.

In Short

  • Receive and manage incoming calls from members regarding benefits and care coordination.
  • Document calls accurately in the designated PHI system.
  • Manage a member caseload requiring short-term care coordination.
  • Work within an interdisciplinary care team to communicate member needs.
  • Perform outreach calls to confirm services are in place.
  • Handle multiple priorities in a fast-paced environment.
  • Support timely communication of member issues.
  • Assist with additional duties as assigned.
  • Maintain effective communication skills both verbally and in writing.
  • Provide high-quality customer service to members.

Requirements

  • Certified Medical Assistant (CMA), Certified Nursing Assistant (CNA), or Registered Medical Assistant (RMA).
  • High School diploma or GED.
  • Experience in a call center or healthcare environment preferred.
  • Familiarity with medical terminology and benefit products.
  • Excellent telephone and organizational skills.
  • Detail-oriented team player.
  • Ability to handle multiple priorities simultaneously.
  • Good grammar, spelling, and punctuation skills.
  • Associate’s degree is a plus.
  • Prior experience in care management is beneficial.
