Overview

The Care Guide Manager leads a team of Care Guides to ensure high-quality, person-oriented care for members navigating complex health and social needs.

In Short

  • Supervise and mentor Care Guide Team.
  • Create high-performance team culture and drive team to achieve all KPIs.
  • Manage schedules, caseloads, performance monitoring, and evaluations.
  • Foster a culture of accountability, innovation, and collaboration.
  • Provide training on workflows/policies, member engagement, and communication strategies.
  • Ensure compliance with regulatory requirements.
  • Monitor and improve care coordination of workflows and outcomes.
  • Conduct audits and reviews to ensure adherence to internal policies/procedures.
  • Identify and act on opportunities for process improvement.
  • Provide onboarding/ongoing training for your team of Care Guides.

Requirements

  • Bachelor’s degree in nursing, Social Work, Case Management, or related field, or equivalent experience.
  • Minimum 3-5 years of experience working in healthcare leading and managing teams.
  • Exceptional communication, collaboration, and critical thinking skills.
  • Experience working in healthcare IT systems/care management software/Electronic Health Records.
