RN, Field Care Manager - Tidewater Region, Virginia - Remote

Humana Healthy Horizons in Virginia is seeking RN, Field Care Managers to perform telephonic and face-to-face assessments and evaluations of the needs and requirements of our long-term services and support (LTSS) and aged, blind and disabled (ABD) member population.

In Short

• Perform telephonic and face-to-face assessments
• Support members in achieving optimal wellness
• Develop individualized care plans
• Identify and resolve barriers to effective care
• Requires minimal supervision
• Travel to members' homes for assessments
• Utilize electronic medical records
• Full-time schedule required
• Participate in Humana's TB screening program
• Must have a valid state driver's license

Requirements

• Active RN license in Virginia
• Two years of healthcare experience
• Experience with case management
• Knowledge of community health resources
• Intermediate to advanced computer skills
• Exceptional communication skills
• Ability to work independently
• Full-time availability
• Preferred: BSN and CCM certification
• Preferred: Bilingual in English and another language
