RN, Field Care Manager (High Intensity Care) - Remote

Join our caring community as an RN, Field Care Manager, where you'll assess and support high acuity members in their homes, facilitating their health and well-being.

In Short

• Perform telephonic and face-to-face assessments of members' needs.
• Guide members and families to appropriate resources.
• Develop individualized care plans for members.
• Monitor member progress towards health outcomes.
• Work autonomously with minimal direction.
• Travel throughout Medicaid regions in Virginia.
• Utilize evidence-based strategies for health management.
• Engage with community health and social service agencies.
• Participate in TB screening program.
• Work Monday to Friday, 8 AM to 5 PM EST.

Requirements

• Active RN license in Virginia.
• Two years of healthcare or case management experience.
• Experience with high acuity populations.
• Intermediate to advanced computer skills.
• Excellent communication and interpersonal skills.
• Ability to work with minimal supervision.
• Valid state driver's license and vehicle insurance.
• Bachelor's degree in Nursing preferred.
• Certification in Case Management preferred.
• Bilingual skills are a plus.
