Overview

The Referral Coordination Manager at Healthy Alliance is responsible for managing the overall performance and day-to-day operations of the Referral Coordination Center, ensuring optimal support for community members and network partners.

In Short

  • Manage the Referral Coordination Center's daily operations.

Requirements

  • Bachelor’s degree required; equivalent experience may be considered.
  • 2-5 years of management or care coordination experience required.
  • Experience in a social care environment preferred.
  • Strong verbal and written communication skills.
  • Knowledge of community social care resources.
  • Excellent time management and multitasking abilities.
  • Effective problem-solving skills and data analysis capabilities.
  • Ability to build and maintain successful teams.
