Overview

The Performance Consultant (PC) is responsible for the successful implementation and execution of terms identified under New York’s 1115 Waiver Amendment’s SCN & HRSN Program and contracts. PCs manage partners’ participation and performance through on-site and remote communication, on-site visits to conduct training, partner risk and needs assessments, and strategic development of performance plans to align partner performance with contract deliverables and Healthy Alliance’s Quality Plan.

In Short

  • Operate as the primary point of contact for all matters related to partner relations.
  • Manage onboarding and training support for Healthy Alliance’s integrated network.
  • Oversee partner contract and performance management.
  • Collaborate with internal cross-functional teams to meet partner needs.
  • Conduct partner risk assessments and resolve issues.
  • Track and communicate monthly partner performance.
  • Maintain knowledge of Medicaid and local transformation.
  • Embody Healthy Alliance’s vision, mission, and goals.

Requirements

  • Bachelor’s degree required. Degree in health, social services, or a related field preferred.
  • Minimum 2+ years of consulting, account management, or health-related experience preferred.
  • Experience with clinical process improvement and performance improvement.
  • Knowledge of social drivers of health and community-based organizations.
  • Excellent organizational and communication skills.
  • Ability to work independently and manage multiple partners.
  • Strong analytical and technical skills.
