Overview

The Manager of Referral Coordination Center is responsible for managing the overall performance and day-to-day operations of Healthy Alliance’s Referral Coordination Center, ensuring seamless service delivery for health-related social needs.

• Manage daily functions of the Referral Coordination Center.
• Support Navigators and Social Care Managers.
• Monitor referrals and productivity expectations.
• Contribute to RCC team meetings and leadership.
• Analyze referral quality data for improvement.
• Provide feedback on service gaps.
• Coach team members on performance objectives.
• Maintain knowledge of Medicaid and social care resources.

Requirements

• Bachelor’s degree required or equivalent experience.
• 2-5 years in management or care coordination.
• Strong communication skills.
• Knowledge of community social care resources.
• Excellent time management skills.
• Effective problem-solving abilities.
• Experience with data analysis for decision-making.
• Ability to build and maintain successful teams.
• Physical ability to perform sedentary work.
