Overview

Ennoble Care is seeking an experienced Full-time Social Worker Care Manager for our house calls department within and around the Griffin, GA region. This hybrid role involves supporting the mental and physical health care of patients with chronic conditions and limited mobility.

In Short

• Support the mental and physical health care of patients.
• Screen and assess patients for mental health and substance abuse disorders.
• Provide patient education about treatment options.
• Monitor patients for changes in clinical symptoms.
• Consult with behavioral health consultants for medication management.
• Facilitate referrals for psychosocial treatments.
• Participate in caseload consultations with psychiatric consultants.
• Document patient progress in EHR systems.
• Facilitate treatment plan changes as necessary.
• Complete assessments and productivity trackers daily.

Requirements

• Minimum of MSW required.
• Preferred Licensure or provisionally licensed.
• Behavioral Health experience and knowledge.
• Must be a licensed driver with an insured automobile.
• Effective communication skills.
• Strong time management skills.
