Overview

The Social Care Manager at Healthy Alliance is responsible for managing Medicaid Members with health-related social needs, ensuring they receive the necessary services and support.

Requirements

• Associate degree in health or social services preferred.
• 5+ years of experience in a clinical or non-profit environment.
• Knowledge of health equity and social drivers of health.
• Excellent communication and presentation skills.
• Ability to build collaborative relationships.
• Detail-oriented and capable of multitasking.
• Proficient in computer skills and technology platforms.
