Social Worker Care Manager - Remote

Overview

Ennoble Care is seeking an experienced Full-time Behavioral Health Specialist for our department supporting patients within and around the Hoboken / Jersey City, NJ area. This position is primarily remote, with the possibility of occasional in-person visits in the future based on patient needs.

In Short

• Support the mental and physical health care of patients on an assigned patient caseload.
• Screen and assess patients for common mental health and substance abuse disorders.
• Provide patient education about common mental health and substance abuse disorders.
• Track treatment response and monitor patients for changes in clinical symptoms.
• Support psychotropic medication management by consulting with behavioral health consultants.
• Provide brief behavioral interventions using evidence-based techniques.
• Participate in caseload consultations with psychiatric consultants.
• Document patient progress and treatment recommendations in EHR.
• Facilitate treatment plan changes for patients not improving as expected.
• Comfortable utilizing Excel and Word for daily assessments.

Requirements

• Minimum of MSW required.
• Preferred Licensure or provisionally licensed.
• Behavioral Health experience and knowledge.
• Must be a licensed driver with an insured automobile.
• Effective communication skills, both verbal and written.
• Strong time management skills.
