Clinical Pharmacist - Remote

Overview

The Clinical Pharmacist at ConcertoCare is responsible for managing drug therapy for patients with chronic conditions, collaborating with healthcare providers, and leading medication safety programs.

• 3 years of pharmacy experience or PGY1 residency required.
• Base salary range of $120,000 to $140,000 with annual bonus.

Requirements

• Active pharmacist license in New York.
• At least 3 years of pharmacy experience or ASHP-accredited PGY1 residency.
• Experience in ambulatory care or managed care.
• Strong communication skills.
• Self-starter with initiative.
• Chronic condition management experience preferred.
• Bachelor's degree or PharmD required.
• Knowledge of managed care and Medicare/Medicaid is a plus.
• Willingness to become licensed in additional states.
