The ConcertoCare Clinical Pharmacist is the expert in drug therapy for the local market, managing chronic conditions and collaborating with healthcare providers.

In Short

• Must be licensed in California.
• Full-time position.
• Execute pharmacist-led disease state management.
• Maintain a patient panel for chronic conditions.
• Collaborate with providers for patient care.
• Lead medication safety programs.
• Organize training for other practitioners.
• Assist in decreasing medication cost utilization.
• Must hold an active pharmacist license.
• Open to becoming licensed in additional states.

Requirements

• Active, unrestricted pharmacist license in California.
• Advanced Practice Pharmacist (APP) preferred.
• 3 years of pharmacy experience or ASHP-accredited PGY1 Residency.
• Experience in ambulatory care or managed care.
• Strong communication skills.
• Motivated self-starter.
• Chronic condition management experience preferred.
• Bachelor's degree or PharmD required.
• Knowledge of managed care and Medicare/Medicaid a plus.
• Willingness to obtain licenses in new markets.
