Clinical Pharmacist - Remote

Overview

We are currently looking for a Clinical Pharmacist (Remote) that will support patients to promote health. The candidate will focus on promoting health as a positive vitality, beyond just the absence of disease.

In Short

• Lead a patient-centered, pharmacist-led disease management program for chronic diseases.
• Collaborate with healthcare providers to ensure optimal patient care.
• Conduct Medication Therapy Management (MTM) and Comprehensive Medication Review (CMR).
• Advise patients on medication use and provide education.
• Analyze patient medication profiles for drug interactions and treatment regimens.
• Apply evidence-based medication guidelines.
• Follow up with patients to ensure progress towards treatment goals.
• Aid in development of clinical algorithms and pathways.
• Assist in coordination of care between healthcare providers and pharmacies.
• Ensure compliance with pharmacy laws and standards.

Requirements

• Doctor of Pharmacy (PharmD) and current pharmacist license.
• Clinical experience in Medication Therapy Management.
• Experience in pharmacist-led Clinical Disease Programs.
• Knowledge of healthcare industry practices and HIPAA regulations.
• Ability to prioritize tasks and manage multiple projects.
• Motivated self-starter with independent decision-making skills.
• Proficient in learning new computer systems.
• Excellent communication and organizational skills.
• Entrepreneurial spirit and solution-oriented mindset.
