Overview

This position involves acting as a dedicated patient support person within an interdisciplinary clinical team, focusing on empowering patients with complex medical needs to actively manage their health.

Requirements

• Bachelor’s degree in healthcare or related field preferred.
• Minimum of 3 years’ experience in chronic condition clinical coaching.
• National Board for Health and Wellness Coaching (NBHWC) Certified preferred.
• Excellent listening, written, and verbal communication skills.
• Empathy and emotional intelligence.
• Ability to build strong relationships with patients.
• Understanding of positive psychology and motivation.
• Comfort with digital technology.
• Ability to critically think and solve problems.
• Flexibility and organization skills.
