Overview

The Health Coach at CVS Health provides telephonic outreach to the Medicare population to enhance medication adherence and health outcomes through education and support.

In Short

  • Conduct assessments of members' health status.
  • Utilize motivational interviewing to engage members.
  • Partner with members to identify integration opportunities.
  • Provide coaching and information to empower health choices.
  • Interpret guidelines to positively impact members' health.
  • Assist members in understanding their health status.
  • Develop proactive actions to enhance health outcomes.
  • Utilize healthcare knowledge to evaluate health risk behaviors.
  • Work collaboratively with managers to overcome barriers.
  • Achieve optimal health status for members.

Requirements

  • 1-3 years of experience with Microsoft Office.
  • 3-5 years of practice experience in relevant fields.
  • Managed Care and Medicare experience required.
  • Must live in EST/CST time zones.
  • Healthcare licensure and/or certifications preferred.
  • Excellent computer skills including MS Office applications.
  • Effective communication and interpersonal skills.
  • Experience in facilitating client positive behavioral change.
  • Background in motivational interviewing and active listening.
  • Associates Degree or equivalent experience.
