Overview

Vesta Healthcare is seeking a Health Coach to work closely with members and caregivers, supporting them in managing chronic conditions and improving health outcomes through personalized coaching and innovative technology.

In Short

• Work with individuals to improve health and manage chronic conditions
• Provide ongoing support and follow-up with members
• Empower members to actively participate in their health outcomes
• Assist members in overcoming health barriers
• Utilize biometric values and motivational interviewing techniques
• Update and maintain care team profiles for coordinated care

Requirements

• Experience in chronic disease management (required)
• Experience educating geriatric populations on lifestyle goals (required)
• Passion for working with underserved populations (required)
• Chronic care management experience (preferred)
• Health Coaching Certification or Bachelor's in Health and Wellness (required)
• At least 1 year of recent health coaching experience (required)
• Bilingual in English and Spanish (required)
• Motivational interviewing training/experience
• Strong computer and software application skills
• Ability to meet individual performance goals
